    Virginia Commonwealth University Partnership for People with Disabilities (VCU-PPD) and the Virginia Department for Aging and Rehabilitative Services (DARS) is conducting a survey of Area Agency on Aging (AAA) clients across Virginia. The aim of the National Core Indicators-Aging and Disabilities (NCI-AD) Adult Consumer Survey is to gather information about the services that AAA clients receive, their needs and preferences, and their quality of life. A random sample of AAA clients across Virginia will be selected for the survey and contacted by VCU-PPD staff. The NCI-AD survey is administered by VCU-PPD staff over the telephone. Participating in the survey is voluntary and completely anonymous. Your voice matters! If you are contacted by VCU-PPD, please consider participating. Prince William Area Agency on Aging Long Term Care Ombudsman Program

Long-term care Ombudsmen are advocates for residents of nursing homes, board and care homes and assisted living facilities. Ombudsmen provide information about how to find a facility and what to do to get quality care. They are trained to resolve problems and help promote and protect residents’ rights.

Residents of Nursing Homes and Assisted Living Facilities have rights that are guaranteed by the Federal Nursing Home Reform Law. The law requires nursing homes to “promote and protect the rights of each resident” and stresses individual dignity and self-determination.

Right to Dignified Existence                           Right to Self-Determination

Right to be Fully Informed                              Right to Raise Grievances

Right of Access to Information                       Rights Regarding Financial Affairs

Right to Privacy                                              Rights During Discharge/Transfer
