Services Assessment

 

 

Confused and overwhelmed? If so, begin with Options Counseling by one of our skilled Supportive Services Staff. Options Counselors can help adults in making decisions based on their preferences, strengths, values and individual circumstances.

Supportive Services Staff work with family members and older adults to assess and evaluate supportive efforts for older adults and their caregivers. Staff take into account personal factors such as age, income, health and disabilities to determine the best available options.

A service assessment is required for the following agency services:

• Home Delivered Meals • Personal Care* • Adult Day Healthcare
