Passport Services are closed on federal holidays.
You are responsible for having all application materials.
Schedule one appointment per application.
Individuals who are 10 minutes late to their scheduled appointment time will need to reschedule.
What will you need to apply for a new passport?
- A DS-11 form completed in black ink
- Original proof of U.S. citizenship (birth/naturalization certificate, old passport)
- Valid, unexpired photo ID
- A 2 x 2 inch passport photo (no glasses)
- Beginning February 2, 2026, Central, Haymarket Gainesville, and Potomac Libraries offer a self-service Passport Photo Booth. The cost is $14.99, payable by credit or debit card only. The Photo Booth is available on a first-come, first-served basis during normal operating hours. The Photo Booth works best for adults and children who can sit up independently.
- Beginning February 2, 2026, Central, Haymarket Gainesville, and Potomac Libraries offer a self-service Passport Photo Booth. The cost is $14.99, payable by credit or debit card only. - A check or money order is physically attached to your application. Debit cards, credit cards, and starter checks are not accepted.
- Passport book: adults - $130; minors 15 and under - $100
- Passport card: adults - $30; minors 15 and under - $15
- Expedite fee: $60
- One check (no starter checks) or cash for Prince William County.
- Acceptance facility fee: $35

For children ages 15 and under, you also need:
- Proof of parental relationship (original birth certificate, adoption papers)
- All custodial parents or legal guardians must appear in person, or the non-appearing parent must submit a notarized DS-3053 and photocopy of ID

RENEWALS:
Please note that if you need to renew your passport and it was issued when you were 16 or older, you can renew by mail using Form DS-82.
Our acceptance facilities cannot process DS-82 applications. For assistance with the DS-82 form, please refer to travel.state.gov or call 1-877-487-2778.
